We are looking for an experienced nurse who is passionate about integrated care and patient flow; and has experience in complex discharge planning, to join our Western Health at Home Community Care Coordination – Paediatric Asthma and Eczema Team, in a part- time, temporary role, until 30 June 2025.

 

The role of the Care Coordinator (CC) is essential to the Western Health at Home teams in providing clients and carers’ with care coordination and management of their chronic illness, and navigation of the Health and Primary care system.  The CC is responsible for comprehensive patient assessments in the home, providing self-management support and coordinating patient healthcare needs across the health care continuum.

As part of this model of care, the Care Coordinators are crucial in identifying services/programs in the community, to ensure that clients get the right service mix to safely support care in the community.

 

To be Successful in this Role you will

  • Have an appropriate qualification in nursing or eligibility for registration with relevant body.
  • Minimum 5 years post graduate experience in Paediatric nursing
  • Have experience in assessment and management of children with asthma and eczema,  discharge planning and care planning
  • Adequate knowledge of community service providers, community healthcare organisations
