Allied Health Assistant - Acute Aged Care at Home Program

About the role:
We are seeking a motivated and skilled Grade 3 Allied Health Assistant (AHA) to join our Acute Aged Care at Home program. This role supports older adults to maintain independence and receive safe, high-quality care in their own homes, helping to prevent hospital admissions and functional decline.
Working under the supervision of an Allied Health professional, you will deliver evidence-based, person-centred care, assisting with therapeutic interventions and related activities in line with individual care plans. You will also collaborate closely with the multidisciplinary team, providing both clinical and non-clinical support to ensure seamless, timely care.
Key responsibilities:
- Deliver safe, effective, and patient-centred care under supervision, promoting functional independence in older adults.
- Collaborate with the multidisciplinary team to support coordinated care in the home environment.
- Communicate effectively with patients, families, and team members to enhance patient outcomes.
About you:
- You are a motivated Allied Health Assistant with a passion and interest in working with older persons within a home environment.
- You have experience as an Allied Health Assistant, preferably in aged care, hospital, or community settings.
- You have strong communication, teamwork, and organisational skills.
- Commitment to delivering high-quality, evidence-based, person-centred care.
