Allied Health Assistant - Grade 2

• Location: Sunshine & Footscray Hospitals (with travel across At Home services, as required)
• Employment Type: Full-Time (80 hours/fortnight), Ongoing (with monthly Accrued Day Off)
• Salary: Grade 2 + Superannuation + Salary Packaging

About the role:

An exciting opportunity exists for an enthusiastic and motivated Allied Health Assistant Grade 2 to join the Rapid Allied Health (RAH) team within Western Health at Home.

Working under the supervision of Allied Health professionals, you will provide high-quality, evidence-based and person-centred care to support patients across a range of home-based and community services. You will assist with therapy programs, support multidisciplinary care, and contribute to innovative models of care that help prevent avoidable hospital presentations and enable patients to remain safely at home.

While primarily based within the Rapid Allied Health service, this role will also provide support across Western Health at Home services, including Emergency Department Care Coordination, Acute Aged Care at Home, GEM at Home, Rehabilitation at Home, Specialised Rehabilitation Programs and the Community-Based Transition Care Program, based on clinical priorities and service needs.

Key responsibilities:

• Assist Allied Health professionals to deliver evidence-based therapy and rehabilitation programs.
• Deliver high-quality, person-centred care while supporting patients to achieve their functional goals.
• Work collaboratively within multidisciplinary teams across hospital and community settings.
• Maintain accurate clinical documentation, workload statistics and patient records.
• Participate in clinical supervision, quality improvement activities and ongoing professional development.

About you:

You are a compassionate and adaptable Allied Health Assistant who enjoys working within a multidisciplinary team and is committed to delivering excellent patient care.

You will bring:

• Allied Health Assistant Certificate III (or equivalent qualification recognised under the Enterprise Agreement).
• A current Victorian Driver Licence.
• Excellent organisational, communication and time management skills.
• Demonstrated understanding of manual handling and safe work practices.
• Computer literacy and the ability to work effectively across community-based healthcare settings (community experience is desirable).
