About the role:
We are seeking a motivated and skilled Grade 3 Allied Health Assistant (AHA) to join the Western Health Community Based Transition Care Program.
This role provides short-term support to older adults in their home and community following hospital discharge. The focus is on delivering interventions that maintain and optimise function, prevent decline, and reduce the risk of readmission or early entry into permanent residential aged care.
The Allied Health Assistant will work under the supervision of the Community Based Transition Care Program team, comprising experienced allied health clinicians and registered nurses trained in targeted interventions across occupational therapy, physiotherapy, and Case management. The role provides both clinical and non-clinical support, contributing to seamless, timely, and patient-centred care.
Key responsibilities:
- Deliver safe, effective, and patient-centred care under supervision, promoting functional independence in older adults.
- Collaborate with the Community Based Transition Care team to support coordinated care.
- Communicate effectively with patients, families, and team members to enhance patient outcomes.
About you:
- A motivated Allied Health Assistant with a passion and interest in working with older persons.
- Experienced as an Allied Health Assistant, preferably in aged care, hospital, or community settings.
- Has strong communication, teamwork, and organisational skills.
- Commitment to delivering high-quality, evidence-based, person-centred care.
