About the role:
Are you a clinical expert in gerontology ready to lead a Home First approach at the front door of our health service?
As the Allied Health Frailty Clinical Lead, you will provide early, expert clinical decision-making for frail older people presenting to the Emergency Department and Short Stay Unit. Working within an acute frailty model and transdisciplinary framework, you will deliver timely, comprehensive assessments that prioritise a Home First approach—preventing functional decline, avoiding unnecessary hospital admissions, and reducing acute length of stay.
You will play a critical role in ensuring proactive, connected safe care for frail adults aged 65+ and Aboriginal and/or Torres Strait Islander people aged 50+. Beyond direct clinical care, this role offers the opportunity to lead innovation, develop advanced practice pathways, and strengthen workforce capability across the organisation.
Key responsibilities:
- Provide senior, upfront frailty assessment and management in the Emergency Department and Short Stay Unit to support safe discharge and optimise patient flow.
- Apply advanced knowledge and expertise in management of frailty and understanding of community and residential care systems to navigate complex care pathways for older people.
- Lead the implementation, and evaluation of evidence-based frailty pathways using improvement science methodologies.
- Drive excellence in care through workforce development, clinical supervision, and the review of advanced practice and credentialing opportunities within the ED Transdisciplinary ED Care Coordination team.
- Work closely with ED Geriatricians, Care Coordinators, and multidisciplinary medical, nursing and pharmacy teams to deliver integrated, patient-centred care.
About you:
- Hold relevant Allied Health tertiary qualification, along with current AHPRA registration and post graduate qualification or working towards in the area of geriatric management is desired
- Have advanced level of clinical expertise in gerontology and acute frailty management, including leadership in complex discharge and transition planning
- Demonstrated experience leading service innovation, quality improvement, and model-of-care redesign in public health
- Strong commitment to Transdisciplinary care coordination models that value role flexibility, collaboration, and shared decision-making.
- Proven ability to build strong partnerships across disciplines and sectors while providing high-quality clinical supervision and leadership.
