About the Role:

We are looking for an experienced Nurse who is passionate about integrated care and patient flow; and has experience in complex discharge planning to join our ACE Inpatient Care Coordination team in a part-time ongoing role.

 

The Advice Coordination and Expertise (ACE) Inpatient Team, supports patient care via the delivery of a service model that is committed to the principles of providing integrated care to patients. Inpatient Care Coordinators are allocated to Medical Units within Western Health and are responsible for facilitating access to community services to ensure a safe and sustainable transition from hospital to home. Western Health is committed to streamlining and responding quickly to patient needs, enabling them to easily navigate the health system. As part of this model of care, the ACE Care Coordinators are crucial in identifying clients and working with other services/programs to ensure that clients get the right service mix to safely support care in the community

 

About You:

  • Have an appropriate qualification in nursing and eligibility for registration with relevant body
  • Have at least 3 years clinical experience in relevant setting including acute and community 
  • Experience and a preference for working in multi-disciplinary teams
  • Experience in assessment of people with complex needs, discharge planning and care planning
