About the Role:
An exciting opportunity currently exists for an Occupational Therapist to work on a fixed term full-time basis (1.0 EFT) (part-time applications considered) within the Emergency Department (ED) Care Coordination team at Western Health, providing:
- Emergency Department Care Coordination as part of a trans-disciplinary model of care
- Occupational Therapy intervention as part of the Rapid Allied Health (RAH) at home service
Key Responsibilities:
The ED Care Coordinator provides support through screening and early identification of at-risk patients presenting to our Emergency Departments and is responsible for the delivery of timely and comprehensive assessment and care coordination of patients with complex needs to facilitate safe and sustainable discharge. This will involve assessment and care planning achieved in collaboration with patients, their carer's and other members of the Emergency Department Clinical team, and appropriate referral, liaison and advocacy with community services where indicated. The service operates between 7:30am and 9:00pm, 7 days a week.
The ED Care Coordinator will also provide support to the Rapid Allied Health team, by providing discipline specific occupational therapy assessment and intervention in patients’ homes to support safe discharge.
About You:
We are seeking applicants who are passionate about care coordination and enjoy working in a changing and fast paced environment and is committed to providing best patient care. To be successful in this role we are looking for a clinician with the following skills and experience:
- Postgraduate qualification in Occupational Therapy and minimum 2 years’ clinical
- experience in relevant acute/community setting.
- Current registration with AHPRA
- Excellent communication and organisational skills
- Current Victorian Driver's Licence
