An exciting opportunity exists for an experienced Nurse or Allied Health Professional to join Western Health's Community Based Transition Care Program in a Case Management role.

 

About the Role:

  • Monday to Friday

The Case Manager role in the Community Transition Care Program (TCP) plays a pivotal role in facilitating the transition of clients from hospital stays to sustainable and supportive long-term living arrangements. They engage closely with clients, families, and the multidisciplinary teams to assess needs and coordinate appropriate community services.

The Case Managers are responsible for ensuring clients receive tailored support and advocate for necessary services to ensure a safe discharge. They contribute actively to assisting with flow and access through the hospital system, providing quality care to complex clients, driving service development and championing quality improvement initiatives through effective communication and comprehensive case management expertise.

 

Skills & Experience:

  • Qualification in Nursing or Allied Health and registration with relevant professional body (as required).
  • Demonstrated experience in Case Management
  • Demonstrated experience working with a multidisciplinary team
  • Experience in complex discharge planning from a hospital setting
  • Experience working with non-acute/community service providers and residential aged care facilities
  • Current understanding of the Transition Care Program and experience with My Aged Care (MAC) to facilitate access to ongoing services, assessments and accommodation pathways
  • Excellent written and verbal communication skills
  • Excellent organisational and time management skills
  • Experience in student supervision
  • Knowledge of and participation in continuous quality improvement processes
  • Commitment to continuing professional development
  • A current Victorian driver's licence
  • Computer literacy
