Clinical Case Coordinator - Dialysis at Home

As a Clinical Case Coordinator in the Dialysis at Home team, you will play a key role in supporting people to manage homebased dialysis safely, confidently, and independently. You’ll work within a multidisciplinary team that delivers innovative, integrated, patient centred care across Melbourne’s West.

Who you’ll work with – you will collaborate with:
- Nurse Unit Manager and Associate Nurse Unit Managers
- Renal Service Manager
- Nursing, allied health and medical staff
- Community service providers, GPs and clinical networks
- Patients, families and carers
Key responsibilities:
In this role, you will coordinate and deliver high-quality, evidence based nursing care for patients undertaking home dialysis. You will provide intake assessment, education, training and ongoing support to patients and their families, ensuring continuity of care and safe practice at home.
You will also:
- Perform comprehensive clinical assessments.
- Identify clinical, social and nonclinical risks and implement risk minimisation strategies.
- Recognise and respond to changes in patient condition.
- Maintain accurate, timely documentation across patient management systems.
- Develop individualised care plans with the multidisciplinary team.
- Schedule timely interventions to prevent complications.
- Provide wound care, medication administration and prosthetic device management (e.g., Tenckhoff catheters, permacaths)
- Use effective conflict resolution skills with patients, families and stakeholders.
- Contribute to service improvement, data collection and KPI monitoring.
- Teach and support peers, graduates, students and patients through tailored education plans
About you:
You are a committed and compassionate Registered Nurse who values person-centred care and collaborative practice. You bring strong clinical capability and a passion for supporting patients to manage dialysis at home.
You will have:
- Bachelor of Nursing (or equivalent)
- Current AHPRA registration as a Registered Nurse
- Minimum one year fulltime (or equivalent) renal nursing experience
- Experience in haemodialysis, peritoneal dialysis or both
- Excellent clinical, organisational and communication skills
- Ability to work effectively within a multidisciplinary team
- Evidence of ongoing professional development
- Commitment to safe, high-quality patient care
- Postgraduate renal qualification (or working towards one) (desirable)
- Ability to adapt teaching methods to diverse learning styles (desirable)
Additional requirements – all employees must:
- Complete police and (if required) Working with Children checks
- Obtain Immunisation Health Clearance
- Comply with Western Health policies, safety procedures and confidentiality requirements
- Uphold relevant legislation and the Victorian Public Sector Code of Conduct
- Support safe interactions with children and young people
- Work across Western Health sites if required
