About the role:

We are seeking an experienced and innovative nurse to join our Nursing at Home team as a Clinical Nurse Consultant (CNC). This exciting opportunity is ideal for a clinician who is passionate about delivering high-quality care beyond traditional hospital settings and supporting consumers to remain healthy, independent and safe in their own homes. 

 

Key responsibilities:

  • Providing expert clinical assessment, triage, care coordination and case management across a variety of community and home-based settings.
  • Responding to clinical concerns escalated by consumers, carers, families, community providers and other healthcare stakeholders.
  • Identifying and managing early signs of deterioration through timely assessment, intervention, education and clinical support.
  • Delivering care through a range of modalities including face-to-face home visits, telehealth, remote patient monitoring and telephone consultation.
  • Supporting hospital avoidance and reducing unnecessary presentations through proactive and evidence-based community care.

 

About you:

  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Experience working in a senior clinical nursing role (Clinical Nurse Specialist or above).
  • Completed or be working towards a Masters degree in Nursing or Post Graduate Diploma in a relevant specialty area.
  • Demonstrated advanced assessment, clinical decision-making and care coordination skills.
  • Experience working autonomously and collaboratively within multidisciplinary team environments.
  • Excellent communication, organisational, time management and problem-solving skills.
