About the Role:

We are looking for an experienced nurse, to join our Residential InReach team as a Clinical Nurse Consultant (CNC). This vital role offers an exceptional opportunity for an experienced nurse who is eager to apply advanced knowledge and expertise across the care continuum, beyond the traditional hospital environment.

 

Key Responsibilities:

  • Leading and responding to patient deterioration and clinical issues raised by Residential Aged Care Staff
  • Identifying and supporting at-risk patients through initiating proactive assessment and treatment plans
  • Facilitating safe hospital transfers/ bridging care for patients returning to Residential Care from Hospital
  • Delivering education and clinical support to multidisciplinary teams
  • Enhancing escalation pathways in collaboration with internal and external stakeholders such as our medical and nursing teams as well as Residential Aged Care facilities, GPs etc, to ensure that the patient needs are met

 

About You:

  • Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve
  • Demonstrated proficient clinical skills to assess and manage clinical deterioration within aged care setting
  • Understanding of hospital avoidance models of care for Residential Aged Care/ Care Outside the Hospital Walls
  • Must have current registration as a Registered Nurse, with the Australian Health Practitioner Regulation Agency and an unencumbered Drivers Licence.
