About the role

Western Health has an exciting opportunity for an experienced and motivated pharmacist to join our talented team as a Grade 2 Clinical Pharmacist – Aged Care. This position is permanent, full-time (80 hours per fortnight) and available from 01/07/2025. 

 

Working under the guidance of both the Senior Pharmacist – Aged Care and Senior Pharmacist – Transitions of Care, you will be part of a growing and supportive team of skilled clinical pharmacists dedicated to delivering high-quality, patient-centred care across our aged care and transitions of care services.

 

This is an excellent opportunity for a pharmacist with demonstrated clinical expertise and a strong interest in aged care and transitions of care to expand their impact in a progressive hospital environment.

 

Key responsibilities:

  • Support the Senior Pharmacist - Aged Care and Senior Pharmacist - Transitions of Care in providing aged care and transitions of care pharmacy services across Western Health.
  • Actively participate in service development, including contributions to departmental projects, research activities, and quality improvement initiatives aimed at improved aged care and transitions of care services.
  • Work collaboratively across the multidisciplinary team to ensure the delivery of high-quality, patient-centred care within the aged care and transitions of care settings.

 

About you:

To be successful in this role, you will:

  • Demonstrate at least 2 years of broad clinical pharmacy experience, ideally with exposure to aged care, general medicine, or transitions of care, and a strong commitment to high-quality, patient-centred care.
  • Apply expert professional knowledge to optimise patient outcomes through safe and effective use of medicines, particularly in vulnerable patient populations at transitional points of care.
  • Have experience in orientating, mentoring, and supervising clinical pharmacy staff, interns and students, and serving as a positive role model for junior team members.
  • Demonstrate enthusiasm in contributing to departmental educational and operational initiatives, including quality improvement activities, audits, and practice-based research.
  • Display excellent communication, interpersonal, and time management skills, with the ability to collaborate effectively within a multidisciplinary team.
