Clinical Pharmacist - Grade 4
Western health is seeking a passionate and dynamic Lead Pharmacist – Western Health at Home to deliver innovative, patient-centred pharmacy services across our rapidly expanding division.
This is a permanent, full-time position (80 hours per fortnight) – available now!
Western Health at home provides leading edge care that empowers our patients to live healthier, more independent lives. Services include Hospital in the Home and Rehab at Home along with more specialised programs like Residential In-Reach, Dialysis at Home and Community Based Transition Care programs. We deliver hospital level care in the comfort of patient’s homes, reducing avoidable hospital stays and supporting sustainable early discharges.
To be Successful in this Role, you will:
- Lead the Western health at home Pharmacy team to deliver high quality, home-based healthcare aligned with best practices and Western Health’s medication governance
- Provide guidance and vision to align pharmacy services with the overarching goals of the Western health at home division
- Develop and implement new models of care, service improvement initiatives and quality assurance activities
- Collaborate with inpatient, ambulatory and community service providers to achieve optimal patient outcomes and reduce hospital admissions
