Clinical Pharmacist - Transitions of Care

About the role:

Western Health is seeking a passionate and experienced Grade 2 Clinical Pharmacist – Transitions of Care to join our Pharmacy team.

This is an exciting opportunity to play a key role in improving medication safety for patients as they move between hospital, home and other healthcare settings. You will work closely with patients, carers and multidisciplinary teams to ensure medication-related information is accurate, clearly communicated and effectively acted upon.

Key responsibilities:

As the Clinical Pharmacist – Transitions of Care, you will provide high-quality, patient-centred clinical pharmacy services with a particular focus on preventing medication-related harm at transitions of care.

• Completing comprehensive medication reconciliation and medicines reviews.
• Identifying, investigating and resolving medication discrepancies.
• Providing evidence-based therapeutic recommendations to optimise medication use.
• Supporting safe and effective discharge planning.
• Providing medication education and counselling to patients and carers.
• Ensuring accurate and timely communication of medication information to GPs, community pharmacists and other healthcare providers.
• Supporting continuity of medication management as patients transition between care settings.
• Working collaboratively with medical, nursing and allied health colleagues to deliver integrated, patient-centred care.
• Contributing to the monitoring, evaluation and continuous improvement of transitions of care practices across Western Health.
• Supporting the implementation of evidence-based medication safety initiatives and quality improvement activities. 

About you: 

To be successful in this role, you will:  

• Provide expert professional knowledge across a broad range of clinical and operational areas to ensure that patient outcomes are optimised.
• Demonstrated strong clinical reasoning and medication management skills.
• Demonstrated experience identifying and resolving medication discrepancies and medication-related risks.
• Demonstrate communication and interpersonal skills, with the ability to engage effectively with patients, carers and healthcare professionals.
• Develop and maintain information technology skills and contribute to the development of innovative Pharmacy Services.
• Work independently and as part of a team and assign priorities to responsibilities to meet time constraints and deadlines.
• Have a valid Australian drivers licence and be able to work across multiple sites including at home settings.
