Clinician - Case Management

About the role: 

Western Health is seeking an experienced Senior Mental Health Clinician (RPN4/OT4/SW4/P3) to join the Intensive Support Program at Maribyrnong Community Team/Saltwater Clinic. The role provides advanced, recovery-oriented clinical care to consumers with complex and high-risk mental health and psychosocial needs. 

Key responsibilities include comprehensive assessment, formulation, evidence-based intervention, risk assessment and management, care coordination, outreach and assertive engagement. The Senior Clinician provides clinical guidance and mentoring to colleagues and students, supports complex decision-making, workload prioritization and safe service delivery, and contributes to multidisciplinary care. 

Applicants require substantial public mental health experience, advanced clinical skills, strong knowledge of the Mental Health and Wellbeing Act 2022, and demonstrated ability to work collaboratively with consumers, families, carers and external services. Experience in trauma-informed, culturally safe and recovery-focused practice is essential, along with strong communication, documentation, organisational and clinical governance skills. A current Victorian driver’s licence and relevant professional registration are required. 

Key responsibilities: 

As an RPN4 within ISP, you will: 

• Provide advanced assessment, evidence-based interventions and recovery-focused care for consumers with complex mental health and psychosocial needs, including outreach and home visits. 

• Develop and monitor collaborative care plans, manage clinical risks, escalate concerns and support complex/high-risk presentations. 

• Coordinate multidisciplinary care, liaise with internal and external services, advocate for consumers and ensure continuity of care. 

• Maintain accurate clinical documentation and records, ensuring compliance with Western Health, ABF, legislative and professional requirements. 

• Provide clinical leadership through mentoring and supporting junior staff/students, contributing to handovers, clinical governance, quality improvement, supervision and professional development. 

About you: 

Essential 

• 5+ years’ nursing or 7+ years’ allied health experience in complex, high-demand clinical settings. 

• Strong knowledge of consumer rights, recovery-focused care, AMHS and evidence-based interventions. 

• Understanding of professional ethics, mental health legislation, the Mental Health and Wellbeing Act 2022, and public mental health policies. 

• Demonstrated clinical leadership, reliability and professionalism. 

• Strong communication, collaboration, problem-solving and negotiation skills. 

• Ability to work with diverse consumers and families using culturally sensitive, recovery-oriented practice. 

• Experience working with primary healthcare and community services. 

• Current Victorian driver’s licence and professional registration.
