About the role 

This role is within our HARP team supporting patients living with diabetes who also have complex needs. You will provide support within our multidisciplinary clinics as well as home visits where necessary.

You will be highly motivated and have excellent communication skills. 

Key responsibilities: 

  • Provide self-management education to complex patients living with diabetes
  • Support patients and team members in the multidisciplinary diabetes and nephrology clinics
  • Support patients undergoing dialysis to manage their changing insulin needs
  • You will work as part of a team, liaising with the inpatient diabetes CNCs to provide holistic care
