About the role:

We are seeking an enthusiastic Enrolled Nurse to join our Transition Care Program (TCP) at Williamstown Hospital. This role offers the opportunity to make a meaningful impact by supporting older adults as they transition from acute care toward greater independence.

 

The TCP Enrolled Nurse is responsible for delivering high‑quality, goal‑focused nursing care within a reablement framework. Working collaboratively with the multidisciplinary team, you will assist patients to optimise their functional abilities, participate in rehabilitation activities, and achieve their individual transition goals. Your contribution will directly support improved patient outcomes, enhanced wellbeing, and safe transitions back into the community or longer‑term care settings.

 

Our TCP service continues to grow, with a strong emphasis on innovation, rehabilitation, and coordinated care planning. We welcome applications from Enrolled Nurses with experience in aged care, subacute care, or rehabilitation, as well as those eager to expand their skills within a supportive and evolving service.

Key responsibilities:

  • Deliver safe, high‑quality, evidence‑based nursing care, recognising changes in patient condition, escalating appropriately, and maintaining accurate clinical documentation.
  • Work collaboratively within the nursing team, accepting delegation within scope, supporting early‑career staff, and role‑modelling behaviours aligned with Western Health’s values and Code of Conduct.
  • Contribute to service improvement by engaging in quality activities, reflective practice, and new initiatives that enhance patient outcomes and consumer experience.
  • Participate in education and learning by supporting new staff and students, updating patient education materials, and sharing improvement activities within the team.
  • Promote evidence‑based practice by staying familiar with organisational policies, contributing to care plan evaluation, and identifying opportunities for clinical practice improvement.

 

About you:

  • Current registration as an Enrolled Nurse with AHPRA, with a completed Diploma of Nursing and IV medication endorsement (if applicable).
  • Demonstrated experience as an Enrolled Nurse in an acute hospital setting, with strong clinical skills and a commitment to safe, person‑centred care.
  • Excellent written, verbal, and interpersonal communication skills, with the ability to work collaboratively within a multidisciplinary team.
  • Strong organisational and time‑management abilities, ensuring efficient and effective delivery of patient care.
  • Demonstrated ability to uphold professional standards, work within scope of practice, and contribute positively to team culture and patient outcomes.
