About the role
As a Registered Nurse in the Geriatric Evaluation Management (GEM) and Dementia Management Unit (DMU) at Sunshine Hospital, you will play a pivotal role in delivering high-quality, person-centred care to older adults with complex needs. Working within a multidisciplinary team, you will be responsible for assessing, planning, and implementing evidence-based nursing interventions to support patients’ recovery, independence, and wellbeing. Key responsibilities:

Deliver evidence-based, person-centred clinical care to patients in the GEM and DMU wards, including assessment, risk minimisation, care planning, and discharge planning.

Collaborate effectively within a multidisciplinary team to coordinate care, provide health education, and support decision-making aligned with patients’ goals.

Demonstrate leadership by supervising early career staff, contributing to peer feedback, and supporting preceptorship and staff development.

Actively participate in quality improvement initiatives, research, and clinical audits to enhance service delivery and patient outcomes.

Maintain accurate, timely, and legally compliant clinical documentation in accordance with Western Health standards and guidelines.
About you:

Endorsed Enrolled Nurse (EEN) (AHPRA) with current registration and proven experience in acute or subacute hospital settings.

A minimum of 1 year experience in aged care nursing.

Demonstrated clinical competence and critical thinking skills, with a commitment to safe, high-quality and person-centred care.

Strong interpersonal, written, and verbal communication skills, with the ability to work collaboratively in a team-focused environment.

Organised and proactive, with effective time management and delegation skills.

Passionate about aged care and dementia management, with a desire to contribute to continuous learning, evidence-based practice, and ongoing professional development.
Desirable criteria:

Experience in providing quality care for dementia patients, with a particular focus on managing behavioural and psychological symptoms of dementia (BPSD)
