General Medicine Discharge Coordinator
Western Health is offering an exciting opportunity for a passionate and experienced Registered Nurse to step into the pivotal role of the General Medicine Discharge Coordinator within the Division of Aged, General Medicine and Subacute Services (AGMS).
With roles based across our Sunshine and Footscray Hospital campuses, this coordinator role is central to driving efficient and timely patient discharges across one of Western Health’s largest and most dynamic inpatient units. In this role, you will collaborate with nursing teams, the access team, multidisciplinary teams, and other key stakeholders to support patient flow and ensure a smooth, safe, and patient-centred discharge process for a diverse range of medical conditions.
As a key contributor to the implementation of the General Medicine redesign work, you will also play an active role in strategic redesign initiatives aimed at enhancing service delivery.
