This is an exciting new opportunity within the Western Health at Home programs within the division of Western Health at Home.

 

Western Health at Home programs include GEM at Home, Rehabilitation at Home, Community Based Transition Care Program, Rapid Allied Health, and Specialised Rehabilitation program. These programs are aimed at providing short term multidisciplinary assessment and intervention to support early hospital discharge, prevent avoidable hospital presentations and admissions, improve function and overall health outcomes and well-being for patients in the Western Health catchment.

 

The Grade 4 Occupational Therapist in Western Health at Home will be responsible for the specialist assessment, care, and management of allocated patient caseload, as well as providing expert contribution to service performance and outcomes. The Grade 4 Occupational Therapist will have a key leadership role within the Western Health at Home teams working collaboratively with key stakeholders and other service providers.

 

 To be Successful in this Role you:

  • Will have at least 10 years’ relevant clinical experience
  • Will have post-graduate qualification in relevant field, or working towards
  • Will have highly developed leadership skills and experience leading teams
  • Will demonstrate highly developed communication and interpersonal skills
  • Will ensure safe and high quality patient care is delivered in line with Western Health values and Live Best Care framework
  • Will contribute to a culture that values ongoing learning and be part of a positive workplace
