Cardiology Services are seeking a dedicated and experienced Heart Failure Nurse Practitioner (HF NP) to join a dynamic team of health professionals working on the SAFE-HF Project at Western Health.

This innovative project aims to reduce hospitalisations for heart failure (HF) patients by providing specialised, collaborative care in both the hospital and primary care settings.

The successful applicant will play a key role in optimising patient outcomes through advanced clinical assessments, patient education, and pharmacotherapy management.
In addition to clinical responsibilities, the HF NP will contribute to the research component of the project, including recruitment, data collection, and reporting. Travel to local primary care practices within the health service catchment area will be required.
This position offers an exciting opportunity to make a meaningful impact on patient care and contribute to a groundbreaking study aimed at improving the heart failure patient journey.

This position is a fixed term (two-year), part-time role supporting the SAFE-HF project with the potential opportunity for ongoing employment at Western Health.

To be successful in this role, you will have:

  • AHPRA endorsement as a Nurse Practitioner 

  • Current clinical experience working with HF patients for at least two years

  • Proven leadership and collaboration capabilities

  • Experience managing quality improvement projects and managing project timelines

  • Up to date knowledge of the Guidelines for the prevention, detection and management of heart failure in Australia and community services available for HF patients
