Overnight Liaison Nurse

 

Western health at Home Team delivers care in the western region community catchment of Western Health. Our Goal is to support patients to return to and/or remain at home following a change in health or functional ability.

The position is required to demonstrate expert knowledge and skills while practicing along the service continuum.  Including responsibility for, but not limited to : triage, liaison, phone support, task based, assessment and case management roles, within the nursing team and as such requires a broad understanding of the scope of the service.
You will be a part of the at Home - Nursing Service Team assisting patients to receive nursing care either at home, a registered facility setting or referral to another @Home service e.g. Allied Health, or hospital.

 

To be successful in the role you
•    A minimum of 3 years' experience in ward based nursing  

•    Have recent clinic skills and general and / or specialised clinical knowledge
•    Have excellent communication skills - both verbal and written
•    Be able to work in a busy environment - be good at prioritising and work ethic 
•    Possess strong clinical nursing skills
•    Are able to demonstrate a high degree of flexibility
•    Demonstrate critical thinking in practice
•    Enjoy working independently
•    advanced IT skills

 Desirable:
•    Experience in working within the community nursing setting with aged care clients  
•    Experience in working within the sub-acute/acute nursing setting with aged care patients
•    Knowledge and understanding of clinical and patient data information systems
