Western Health is seeking a passionate and motivated Nurse Practitioner Candidate to join our Hospital in the Home (HiTH) team. This is a unique opportunity to contribute to innovative models of care that deliver the right care, in the right place, at the right time.
About the Role
As a Nurse Practitioner Candidate, you will work within the acute HiTH program, supporting patients to receive hospital-level care in the comfort of their own home.
This role offers a unique blend of autonomy and collaboration, with opportunities to contribute to service innovation, clinical education and research.
You will:

Provide advanced clinical nursing care to acutely unwell patients in the community

Work independently and alongside a dynamic team of medical, nursing, allied health and external providers
- Prescribe medications, order diagnostic investigations, and make specialist referrals (as per scope and progression)

Lead and support nursing staff in complex clinical situations

Contribute to research, quality improvement, and service development
About You
You're a highly motivated clinician with a strong drive to provide exceptional, patient-centred care. You thrive in a fast paced, autonomous environment and are ready to develop your advanced practice capabilities.
You have:

Current enrolment in an NMBA-accredited Nurse Practitioner Master's program

Clinical experience in areas such as geriatric care, chronic disease management, and acute illness care

A commitment to best care, safety, and patient empowerment

Highly developed critical thinking, organisational, and clinical decision-making skills

A team approach to meeting workforce, quality, access, and financial KPI's

Western Health also provides the primary healthcare service at the Dame Phyllis Frost Centre, a women’s prison located in Ravenhall.
