Western Health is seeking a passionate and motivated Nurse Practitioner Candidate to join our Hospital in the Home (HiTH) team. This is a unique opportunity to contribute to innovative models of care that deliver the right care, in the right place, at the right time.

About the Role

As a Nurse Practitioner Candidate, you will work within the acute HiTH program, supporting patients to receive hospital-level care in the comfort of their own home. 

This role offers a unique blend of autonomy and collaboration, with opportunities to contribute to service innovation, clinical education and research.

You will: 

  • Provide advanced clinical nursing care to acutely unwell patients in the community

  • Work independently and alongside a dynamic team of medical, nursing, allied health and external providers

  • Prescribe medications, order diagnostic investigations, and make specialist referrals (as per scope and progression)
  • Lead and support nursing staff in complex clinical situations 

  • Contribute to research, quality improvement, and service development

About You

You're a highly motivated clinician with a strong drive to provide exceptional, patient-centred care. You thrive in a fast paced, autonomous environment and are ready to develop your advanced practice capabilities. 

You have: 

  • Current enrolment in an NMBA-accredited Nurse Practitioner Master's program

  • Clinical experience in areas such as geriatric care, chronic disease management, and acute illness care

  • A commitment to best care, safety, and patient empowerment

  • Highly developed critical thinking, organisational, and clinical decision-making skills

  • A team approach to meeting workforce, quality, access, and financial KPI's
