The Division of Aged, General Medicine and Subacute Services (AGMS) has an exciting opportunity for an enthusiastic and motivated registered nurse ready to step into a dynamic leadership role.
Our Bed-Based Transitional Care Program ward is a 30-bed ward that is dedicated to delivering exceptional care in a dynamic and nurturing environment, focused on enhancing the recovery and independence of older people post-hospitalisation and supporting their journey back into the community or on to Residental Aged Care.
We’re looking for a dynamic nurse leader with proven experience and motivation with a passion and strong commitment for caring for the older person to lead our team. Key responsibilities:
You will play a pivotal role in implementing and ensuring compliance with the Strengthened Aged Care Standards, actively shaping the future of aged care at Western Health. This includes:
- Leading the integration of the Aged Care Quality Standards into daily practice
- Collaborating across Western Health committees to ensure alignment with national aged care reforms
- Championing initiatives that promote safety, dignity, and quality of life for older people
- Driving continuous improvement through evidence-based practice and audit processes
- Provide effective leadership and operational management of the ward team
- Ensure delivery of high-quality care aligned with the principles of the Aged Care Quality Standards
