Our specialist Bed-based Transitional Care Program (TCP) team plays a vital role in providing high-quality care in a supportive and dynamic environment to optimise the functioning and independence of older people after an acute hospital stay.

 

TCP is a speciality 30-bed ward, located at Williamstown Hospital, that facilitates a continuum of care for older people who have completed their hospital episode, including acute and subacute care and who may benefit from restorative care and more time and support their recovery after a hospital stay, remain independent in their own home or finalise ongoing care arrangements.

 

We are seeking a motivated and experienced Nurse Unit Manager (NUM) with a passion for caring for older people, to lead our team. This is an exciting leadership opportunity to inspire and manage a dedicated group of healthcare professionals while fostering a culture of continuous improvement, teamwork, and excellence in patient care.
