We are seeking an experienced Senior Physiotherapist to join our GEM at Home team at Western Health. This leadership role offers the opportunity to deliver exceptional physiotherapy care to patients with complex and chronic conditions, primarily in a home-based setting.

 

Key Responsibilities:

  • Provide expert clinical care to older patients with complex conditions, ensuring evidence-based practice.
  • Work collabratively in a skilled multi-displinary team.
  • Lead and mentor Grade 2 physiotherapists and Allied Health Assistants , fostering a culture of  support and continuous learning.
  • Oversee service delivery and quality improvement initiatives to optimise patient care and team performance.
  • Provide clinical governance and leadership within the GEM at Home team and across other Western Health at Home teams as required.

 

About GEM at Home:

Western Health’s GEM at Home service is a bed-substitution model that supports older people with complex, chronic or multiple health conditions to be assessed, treated and managed in the comfort of their home rather than in hospital.

The service utilises a time-limited multidisciplinary care team approach, and aims to assist patients to achieve their care goals, and best possible level of health and function, while reducing the time spent in, and risks associated with, hospitalisation.
