Physiotherapist - Grade 2 (Aged & Complex Care)

About the Role:
Our Aged and Complex Care stream has a strong focus on patient and family centred care, implementing evidence-based therapy to achieve excellent patient outcomes that support independence, community participation, and quality of life.
The Grade 2 Aged and Complex Care rotation program offers rotations in Acute Aged Care, Geriatric Evaluation and Management (GEM), and Bed Based Transition Care Program (TCP). The Aged and Complex Care team also provides physiotherapy service to our Dementia Management Unit, and Inpatient Palliative Care Unit. Rotations are approximately 9 months duration and across Footscray, Sunshine, and Williamstown campuses.
In the Acute Aged Care rotations, you will use your skills in discharge planning, person-centred care, and collaboration with the Multidisciplinary Team to deliver Best Care to a variety of patient cohorts including acute and chronic respiratory illnesses, heart failure, post falls management, functional decline, frailty, and other complex co-morbidities. The Acute Aged Care team at Footscray also contribute to the General Medicine physiotherapy service.
Working in GEM or Bed Based TCP will enable you to use and develop your rehabilitation and discharge planning skills. The clinical caseload includes a variety of aged and complex care patient cohorts across neurological, orthopaedic, cardiorespiratory and general frailty, falls, and deconditioning clinical presentations. Our GEM service at Sunshine Hospital includes Aquatic Physiotherapy in our on-site hydrotherapy pool and you will have the opportunity to provide this therapy.
Learning and development are integral in the culture of our department, and we will support you to build you clinical, supervision and leadership skills. We will encourage you to participate in our Research and Quality program and contribute to our stream portfolios and department committees. Physiotherapists at Western Health are also encouraged in career development and planning through the Allied Health workforce development program.
To be Successful in this Role you will:
- Have well developed clinical skills for working in the acute medical or subacute clinical areas, with a particular focus on complex discharge planning and aged related conditions. Hospital experience is essential.
- Have an interest in supervision and training physiotherapy staff and students
- Be willing and enthusiastic to contribute to Research and Quality activity
- Demonstrate excellent communication and interpersonal skills
- Work collaboratively with the Physiotherapy and Multidisciplinary teams
- Ensure safe and high-quality patient care is delivered in line with Western Health values and Live Best Care framework
- Contribute to a culture that values ongoing learning and be part of a positive workplace
