About the role:
Rehab at Home is a bed substitution program that accelerates the discharge of neurological, orthopaedic, and general medical patients admitted to hospital. The program provides intensive, multidisciplinary rehabilitation in the home and community setting immediately following discharge.
The Rehab at Home Senior Physiotherapist plays a pivotal role in managing and delivering physiotherapy services for patients receiving home-based rehabilitation care. Combining clinical expertise with leadership, this role focuses on improving patient outcomes while driving team development. As a senior member of the team, the position involves effective collaboration with key stakeholders during discharge planning, ensuring that realistic and achievable goals are set for patients' rehabilitation journeys.
Key responsibilities:
- Advanced clinical expertise in physiotherapy
- Extensive experience in interdisciplinary, goal directed rehabilitation
- Excellent communication, negotiation, and interpersonal skills
- Strategic thinking with a focus on quality improvement
- Ability to manage complex caseloads and work autonomously in a home-based setting
About you:
- Expertise in delivering assessments and clinical interventions, including problem-solving complex clinical scenarios
- Demonstrated specialist knowledge in clinical reasoning and discharge planning
- Proven leadership experience, including managing and delivering service teams to achieve KPIs and quality outcomes
- Prioritise and balance demands between providing direct clinical care and engaging in non-clinical tasks such as quality improvement initiatives, program coordination, and strategic development.
