Western Health is seeking a compassionate and motivated Psychosocial Support Worker to join our Hospital Outreach Post Suicidal Engagement (HOPE) team.
The HOPE program provides intensive, short-term psychosocial support to people following a suicide attempt or suicidal crisis. Working alongside a Mental Health Clinician, you will support consumers to build resilience, strengthen protective factors and reconnect with the services, supports and community resources that matter most to them.

Key Responsibilities:
As a HOPE Psychosocial Support Worker, you will:
- Provide practical, recovery-focused psychosocial support to consumers following a suicidal crisis.
- Work collaboratively with Mental Health Clinicians to deliver coordinated care and support planning.
- Build strong therapeutic relationships with consumers, families and carers.
- Support consumers to access housing, financial, employment, education and community supports.
- Advocate for consumers and promote recovery, hope and social connectedness.
- Work closely with hospitals, GPs, community agencies and other support services to ensure seamless transitions of care.
- Contribute to service evaluation and ongoing quality improvement.
About You:
You are someone who is passionate about supporting people through recovery and understands the importance of compassionate, person-centred care.
You will have:
- Qualifications and/or experience in community services, mental health, alcohol and other drugs, allied health, family support or a related human services field.
- Experience supporting people with complex psychosocial needs.
- Excellent communication and relationship-building skills.
- The ability to work collaboratively within a multidisciplinary team.
- Strong organisational and time management skills.
- A commitment to recovery-oriented and trauma-informed practice.
- A current Victorian Driver Licence.
