Registered Nurse - Residential In Reach
- Location: Sunshine Hospital
- Employment Type: Part-Time (28 hours/week), Fixed Term (until June 2027)
- Salary: RN Grade 2 + Superannuation + Salary Packaging
About the role:
The Registered Nurse, as a key member of the Residential In-Reach (RIR) team, delivers safe, person-centered clinical care and supports continuity of care for residents in partner Residential Aged Care Facilities (RACFs). The role provides onsite assessment and escalation support in collaboration with senior Clinical Nurse Consultants, including participation in two-person and procedure-based visits. The Registered Nurse builds capability across the aged-care workforce by delivering education, micro-teaching, simulation sessions, and developing clinical resources to strengthen recognition and response to deterioration.
Key responsibilities:
- Work collaboratively within the nursing team model of care using supervision, coordination, consultation and referrals
- Using evidence-based practice, participate in evaluation of the progress of individuals and groups towards expected outcomes and reformulation of care plans
- Provide education/promotion of Residential In-reach to such audiences such as wards and Residential Aged Care Facilities
- Participate in new initiatives to enhance consumer outcomes and experience, or staff employment outcomes and experience
About you:
- Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve
- Demonstrated proficient clinical skills to assess and manage clinical deterioration within aged care setting
- Understanding of hospital avoidance models of care for Residential Aged Care/ Care Outside the Hospital Walls
- Must have current registration as a Registered Nurse, with the Australian Health Practitioner Regulation Agency and an unencumbered Drivers Licence.
