About the role:

Our Subacute Services at Western Health are seeking highly skilled and motivated Registered Nurses to join our team. Our service provides a diverse range of programs including Palliative Care, Transitional Care Program, Geriatric Evaluation and Management (GEM) and Rehabilitation services.

 

Subacute Services at Western Health operates across multiple sites and is currently offering a range of exciting opportunities for experienced and compassionate Registered Nurses to join our multidisciplinary team.

 

Key responsibilities:

  • Using evidence-based practice, participate in evaluation of the progress of individuals and groups towards expected outcomes and reformulation of care plans
  • Be familiar with local and organisational policies, procedures, and guidelines
  • Contribute to adverse event investigations and undertake individual and team-based reflective practice
  • Participate and contribute to improvement of policies, procedures and protocols and identify
  • Participate in ongoing learning opportunities and complete all required Mandatory competencies
  • Being able to work collaboratively.

  • Registration as a Registered Nurse in Australia

Registered Nurse – Rehabilitation Ward

About the role:

The Rehabilitation ward is a 30-bed unit located at Sunshine Hospital. As a Registered Nurse on the Rehabilitation Ward, you'll play a crucial role in facilitating the recovery journey of our patients. You'll work closely with multidisciplinary team to deliver comprehensive care tailored to each patient's needs.

 

About you:

  • 3 years’ experience as a Registered Nurse
  • Demonstrated ability to practice collaboratively as part of an inter-professional health care team with effective communication, both written and verbal
  • Demonstrated evidence of undertaking professional development activities to maintain and enhance nursing expertise

 

Desirable criteria

  • Rehabilitation nursing experience

 

Registered Nurse – Ward 2B (GEM - Geriatric Evaluation and Management)

  • Location: Sunshine Hospital
  • Employment Type: Part-Time (64 hours/fortnight), Ongoing

 

About the role:

Ward 2B is a 24-bed GEM (Geriatric Evaluation and Management) ward located at Sunshine Hospital. We are a dynamic and passionate team comprising of dedicated multi-disciplinary team who strive to ensure that we work alongside our patients and families to achieve patient focused outcomes.

 

About you:

  • Must be passionate about improving outcomes for older people in hospital
  • Have excellent written and verbal communication skills
  • Should have recent subacute/medical nursing experience for at least 2-3 years in a leadership role
  • Hold a portfolio or experience in portfolio work with change management experience
  • Work well in a multidisciplinary team care-based model
