Grade 3 Social Worker- Full time, permanent opportunity (Western Health at Home)

 

Western Health is seeking a highly motivated and dynamic Grade 3 Social Worker to join our Western Health at Home multidisciplinary teams.

This role works as part of well-supported and innovative teams providing home-based and centre-based care to Western Health at Home patients.

The purpose of this position is to:

 

  • Provide best practice patient centred clinical care to clients within the allocated caseload admitted to Western Health at Home programs.
  • To provide specialized social work support to patients that are experiencing crisis, trauma, or other complex social and health-related issues. The role involves case management, advocacy, and direct support, as well as supervision and leadership within the social work team.
  • Assess and provide therapeutic interventions for patients facing a range of challenges, such as family violence, child protection concerns, mental health issues, and complex issues.
  • Contribute to the development of Western Health at Home services.

This role offers the opportunity for development in leadership and creative problem solving, with a mixture of work across admitted and non-admitted programs within Western Health at Home.

 

 To be successful in this role you:

 

  • Demonstrate commitment to provision of social work support and intervention
  • Have extensive knowledge and experience in patient care in community setting
  • Have strong collaboration and time management skills
  • Have experience working within multidisciplinary teams
  • Will demonstrate highly developed communication and interpersonal skills
  • Have a valid Victorian Drivers Licence
