About the role:
Western Health is seeking a dynamic and experienced health leader to take on the pivotal role of Service Manager – ED Care Coordination, Rapid Allied Health & Psychology at Home. This position offers the opportunity to lead and influence innovative, patient-centred models of care across acute and community settings within one of Victoria’s most progressive health services.
As part of the Western Health at Home Division, this role provides strategic and operational oversight for a suite of integrated programs focused on improving patient flow, supporting timely and safe discharge from hospital, and preventing avoidable presentations and admissions.
Operating across our Footscray, Sunshine and Williamstown campuses, the Service Manager will lead a multidisciplinary workforce committed to delivering responsive, collaborative, and high-quality care in partnership with patients, families, and community providers.
Key responsibilities:
- Provide operational leadership and workforce management across ED Care Coordination, Rapid Allied Health, and Psychology at Home services
- Oversee service planning, performance, budget and resource allocation
- Drive team development through supervision, coaching, training, and performance review
- Lead and support continuous improvement, innovation, and research initiatives
- Strengthen internal and external partnerships to enhance integration and continuity of care
- Ensure service delivery aligns with Western Health’s Best Care principles and organisational priorities
Abou you:
- A tertiary qualification in an Allied Health discipline and registration with AHPRA.
- Demonstrated experience in healthcare leadership, ideally within rehabilitation or community-based models of care.
- Exceptional skills in communication, collaboration, and strategic thinking.
- Proven ability to drive service innovation and lead teams through change.
- Commitment to safe, person-centred care and quality improvement.
- A valid Victorian Driver Licence for community service delivery
