About the role:

Are you a forward-thinking Allied Health leader looking to drive impactful change in rehabilitation care? This is an exciting opportunity to lead the Specialised Rehabilitation Program (SRP) at Western Health — a dynamic, community-based outpatient service focused on optimising recovery and helping people live more healthy days at home.

 

As Service Manager, you will play a pivotal role in shaping the strategic direction, operational excellence, and team culture of a multidisciplinary program delivering high-quality, short-term rehabilitation after hospitalisation, illness, or injury.

 

The Specialised Rehabilitation Program provides timely, goal-oriented care that supports functional recovery, promotes independence, and reduces hospital readmissions. Delivered by a collaborative Allied Health team, this service enhances patient outcomes through coordinated, evidence-based rehabilitation in the community setting.

 

Key responsibilities:

  • Provide strategic and operational leadership, ensuring service delivery aligns with Western Health strategic plan and Best Care principles.
  • Drive continuous service improvement, identifying opportunities to enhance patient experience, access, and outcomes.
  • Lead and support a skilled multidisciplinary team, fostering a positive, high-performance culture.
  • Oversee daily operations including workforce planning, resource management, and service coordination.
  • Ensure high standards of clinical governance, patient safety, and compliance.
  • Facilitate staff development and mentorship, supporting growth and capability across the team.
  • Engage stakeholders and strengthen partnerships across acute, community, and primary care settings.

 

About you:

  • A tertiary qualification in an Allied Health discipline and registration with AHPRA.
  • Demonstrated experience in healthcare leadership, ideally within rehabilitation or community-based models of care.
  • Exceptional skills in communication, collaboration, and strategic thinking.
  • Proven ability to drive service innovation and lead teams through change.
  • Commitment to safe, person-centred care and quality improvement.
  • A valid Victorian Driver Licence for community service delivery
