Subacute and Non-acute Access and Pathways (SNAP) Clinician

About the role:

Western Health is offering an exciting role in the Division of Aged, General Medicine & Subacute Services for a skilled Clinical Nurse Consultant (CNC) or Allied Health Senior Clinician to join the Subacute and Non-acute Access and Pathways (SNAP) service.

The SNAP Service is a pivotal service at Western Health ensuring patients have access to subacute and non-acute pathways which facilitate the provision of the right care at the right time and in the right place.  This includes screening and comprehensive assessment of patients to facilitate pathways out of acute care and into subacute care. 

We are seeking applicants with highly developed communication and interpersonal skills. Key responsibilities:

• A professional clinical role model who combines high emotional intelligence and clear communication to mentor multidisciplinary teams, drive consumer-focused innovation, and provide decisive direction in complex health environments.
• Delivers high-level assessments, interventions, and rapid-response care planning as a primary resource, while managing clinical risk, steering multidisciplinary working groups, and aligning specialty practice with National Safety and Quality Health Service Standards.
• Proactively drives local and organizational practice improvements by auditing service standards and applying current research evidence to clinical care.
• Continuously reviews clinical outcomes, policies, and practices against the latest evidence to investigate adverse events, drive process redesign initiatives, and align team outcomes.
• Committed to advanced professional development by sharing knowledge from seminars, mentoring less experienced staff, establishing learning networks, and designing evidence-based education that empowers both multidisciplinary teams and patients to make informed care choices.

About you:

• Current registration as an Allied Health Professional or as a Registered Nurse with AHPRA.
• A completed postgraduate qualification (certificate/diploma) in nursing.
• A commitment to Western Health Care’s Values, Compassion, Accountability, Respect, Excellence, Safety.
• Experience working in a senior clinical role and a strong capability for providing healthcare education and mentoring.
• Ability to work autonomously with minimal supervision, while actively collaborating across multidisciplinary teams to deliver best patient care.

• Equipped with high-level verbal and written communication skills, alongside strong negotiation and analytical problem-solving abilities.
