About the Role:
Are you an experienced mental health clinician looking to take the next step into a leadership role?
Western Health is seeking a Team Leader – Case Management to provide clinical and operational leadership within our Adult Community Mental Health and Wellbeing Team based in Melton.
This is an exciting opportunity to lead a skilled multidisciplinary team supporting consumers with ongoing and complex mental health needs within a recovery-oriented, trauma-informed and contemporary service.
Key Responsibilities:
As Team Leader, you will oversee the day-to-day operations of the case management function, ensuring safe, high-quality and timely care is delivered to consumers. You will work closely with clinicians, psychiatrists and other service functions including Intake, ISP and ACIS to support continuity of care, service flow and positive consumer outcomes.
Key responsibilities include:
- Providing clinical leadership, supervision and support to clinicians
- Oversight of caseloads, allocations and service demand
- Supporting clinicians to manage complexity, risk and competing priorities
- Facilitating smooth transitions across community service functions
- Monitoring documentation, KPIs and quality activities
- Supporting workforce development, reflective practice and staff wellbeing
About You:
You are an experienced mental health clinician who:
- Has extensive community mental health experience
- Has demonstrated leadership or senior clinician experience
- Is confident supporting staff managing complex presentations and clinical risk
- Understands the Mental Health and Wellbeing Act 2022
- Values collaboration, recovery-oriented practice and positive team culture
- Brings a practical, organised and solution-focused approach to leadership
