About the role:

Western Health is seeking an experienced and motivated Team Leader to join our innovative Western Health Mental Health at Home (WHMH at Home) service — a contemporary model of care delivering acute mental health treatment within the home environment.

 

WHMH at Home is a 24/7 service providing the equivalent of ten acute mental health beds in the community. Underpinned by the Hospital in the Home (HITH) model of care, the service delivers intensive, recovery-oriented support to consumers experiencing acute mental health crises who would otherwise require inpatient admission.

 

As Team Leader, you will play a key operational and clinical leadership role supporting the day-to-day functioning of the service. Working closely with the Program Manager, you will coordinate operational requirements including referral triage, staffing oversight, roster management, workload allocation, patient flow, quality improvement and clinical coordination to support safe, responsive and consumer-centred care delivery.

 

A core component of the role is the acceptance, screening and triage of referrals into the service, ensuring timely and clinically appropriate access to care in collaboration with internal and external stakeholders.

 

This role offers an exceptional opportunity to contribute to an innovative service redefining how acute mental health care is delivered — supporting consumers to recover safely and confidently within their own homes while strengthening community-based alternatives to hospital admission.

 

Key responsibilities:

  • Support the day-to-day operational management and coordination of the WHMH at Home service.
  • Coordinate intake, screening, acceptance and triage of referrals, ensuring timely and clinically appropriate responses.
  • Develop, manage and publish staff rosters while supporting safe staffing profiles and skill mix requirements.
  • Coordinate planned and unplanned leave including annual leave, personal leave and roster replacement requirements.
  • Allocate clinical workload and support effective patient flow, admissions, transitions and discharge planning.
  • Provide operational and clinical support during periods of workforce shortage, increased service demand or clinical escalation, including supporting outreach functions where required.
  • Support achievement of service Key Performance Indicators (KPIs), operational priorities and Best Care outcomes.
  • Contribute to quality improvement initiatives, audits, service development activities and operational reporting.
  • Provide leadership, supervision, mentoring and support to nursing and multidisciplinary staff.
  • Foster a positive workplace culture focused on collaboration, psychological safety, continuous improvement and recovery-oriented practice.
  • Act as a delegate for the Program Manager during periods of leave or absence as required.

 

About you:

  • You are an experienced and adaptable mental health clinician with strong operational and leadership capability. You thrive in fast-paced environments, demonstrate sound clinical judgement and are passionate about delivering safe, recovery-oriented and consumer-centred care within community settings.
  • You are highly organised, proactive and collaborative, with the ability to coordinate competing operational demands while supporting staff wellbeing, service quality and positive consumer outcomes.
  • Current registration as a Registered Nurse with AHPRA and postgraduate qualifications in Mental Health Nursing (or equivalent).
  • Extensive experience in acute and/or community mental health settings.
  • Demonstrated experience coordinating teams, operational workflows and clinical service delivery in dynamic environments.
