About the Role:

An exciting opportunity currently exists for an experienced Grade 2 Allied Health Professional to join the Emergency Department (ED) Care Coordination team at Western Health on a fixed-term, part-time basis (0.8 EFT) working across Western Health’s multiple ED’s on 7-day rotating roster (including both AM and PM shifts).

 

This role delivers Emergency Department Care Coordination within a transdisciplinary model of care, supporting timely, patient-centred assessment, intervention, and discharge planning for patients presenting to the Emergency Department, with a focus on reducing re-presentation.

 

The ED Care Coordinator supports patients through screening and early identification of at-risk individuals and is responsible for providing timely, comprehensive assessment and care coordination for patients with complex needs, with the aim of facilitating safe and sustainable discharge and reducing re-presentation to the Emergency Department.

 

Key Responsibilities:

  • Assessment and care planning in collaboration with patients, their carers and members of the wider Emergency Department team
  • Referral, liaison, and advocacy to appropriate community services where indicated
  • Supporting continuity of care across the acute and community interface
  • Assisting with achieving ED targets and KPIs
  • Supervision of Students Grade 1s and Allied Health Assistants

 

The service operates between 7:30am and 9:00pm, seven days a week.

The ED Care Coordinator will also provide support to the Rapid Allied Health team by delivering discipline-specific occupational therapy assessment and intervention in patients’ homes to support safe discharge.

 

About You:

To be successful in this role, you will have:

  • A minimum of two years’ clinical experience in an Emergency Department setting
  • Experience in a relevant acute and/or community setting
  • Strong discharging planning and problem-solving skills
  • Knowledge of appropriate community services and supports
  • Current registration with AHPRA (where applicable)
  • Excellent communication, organisational, and time management skills
  • Current Victorian Driver's Licence
